Registration fee includes continental breakfast both days, buffet lunch and Networking Reception on Monday, and a boxed lunch on Tuesday.

Payment Information

~Checks/money orders must be in U.S. dollars and made payable to SASED d/b/a Illinois PBIS Network.

~Credit card payments will only be accepted with online registrations. NOTE: If paid by credit card, the name appearing on your credit card billing statement will be "Illinois PBIS Network."

~Purchase orders will also accepted. Be sure to include the conference name/date and the names of all registrants on your purchase order and fax to 630-325-2605.

~All fees are due on or before the first day of the conference, September 8, 2014.

~Mail payment to: ISTAC - Illinois PBIS Network, 550 Quail Ridge Drive, Westmont, IL 60559.

Refund Information

~Cancellations received by Thursday, August 28, 2014 will be refunded at the full amount paid.

~Cancellations received between August 29 - September 7, 2014 will be refunded the amount paid less $75 to cover non-refundable expenses.

~No refunds will be provided after September 7, 2014.

~If you do not cancel and do not attend the event, you are responsible for full payment.

~Substitutions may be made at anytime.
