Once each school year, the Illinois PBIS Network formally recognizes schools that demonstrate implementation and sustainability of the PBIS process through measurable criteria at the Tier 1/Universal, Tier 2/Secondary, and Tier 3/Tertiary levels. Schools that want to be recognized for their implementation of PBIS for the 2012-13 school year are required to submit an application, along with supporting documentation, for one of the six levels of school recognition: Start-Up, Emerging, Implementing, Implementing Plus, Fully Implementing, or Exemplar.

The Illinois PBIS Network Phases of Implementation (PoI) is the primary tool by which implementation is measured, and is an integral part of the application process. The PoI has been updated to v4.1 for the 2012-13 school year to clarify the benchmarks around family engagement, Tier 2 and Tier 3, and ensure benchmarks are aligned with effective implementation in each phase. See instructions on the PoI Rubric for the procedures associated with this self-assessment tool, including the roles of the External Coach and the Technical Assistance Coordinator. It is recommended that the PoI be used initially in November and updated in April for effective self-assessment and action-planning.

Depending on the level of recognition a school is seeking, other self-evaluation and/or outside evaluation tools used in PBIS implementation will also need to be submitted with the application. Complete details are indicated on the applications for each level.

Key Updates to Recognition Application Criteria for 2012-13:

Requirements

Start-Up: Beginning the process of school-wide PBIS implementation

Requirements:

·         Use and submit the PoI at least once yearly

·         Demonstrate PBIS implementation by fulfilling one or more criterion on the most recent PoI at the following phase: Tier 1, Phase I

·         Participate in Tier 1 Training.

Emerging: Building the foundation for school-wide PBIS implementation

Requirements:

·         Use and submit the PoI at least once yearly

·         Demonstrate PBIS implementation by fulfilling all criteria on the most recent PoI at the following phase: Tier 1, Phase I

Provide the following artifacts (see application for details) demonstrating implementation of each relevant phase in the PoI:

o    Use and submit a Multi-Tiered Action Plan (MAP)

o    Use and submit the Team Implementation Checklist (TIC), or score 70 or higher on the Benchmarks of Quality (BoQ), or score 80/80 on the School-wide Evaluation Tool (SET)

o    Use and submit the Self-Assessment Survey (SAS)

Implementing: Maintaining the school-wide PBIS foundation and building the foundation for Tier 2 Interventions

Requirements:

·         Use and submit the PoI at least once yearly

·         Demonstrate PBIS implementation by fulfilling all criteria on the most recent PoI at the following phases: Tier 1, Phase I and II, and Tier 2, Phase I

Provide the following artifacts (see application for details) demonstrating implementation of each relevant phase in the PoI:

o    Use and submit a Multi-Tiered Action Plan (MAP)

o    Implement and submit a Daily Progress Report (DPR) for Tier 2/Secondary Interventions

o    Use and submit a Tier 2/Tier 3 Interventions Tracking Tool which includes students with simple CICO or SAIG secondary interventions

o    Score 70 or higher on the Benchmarks of Quality (BoQ), or score 80/80 on the School-wide Evaluation Tool (SET)

o    Use and submit the Self-Assessment Survey (SAS)

Implementing Plus: Sustaining school-wide PBIS implementation, maintaining Tier 2 Interventions, and building the foundation for Tier 3 Interventions

Requirements:

·         Use and submit the PoI at least once yearly

·         Demonstrate PBIS implementation by fulfilling all criteria on the most recent PoI at the following phases: Tier 1, Phase I, II, and III, Tier 2, Phase I and II, and Tier 3, Phase I

Provide the following artifacts (see application for details) demonstrating implementation of each relevant phase in the PoI:

o    Provide a description of parent or family involvement documenting that one or more family members are active members of school-based PBIS teams (target is participation in 50% of meetings)

o    Use and submit a Multi-Tiered Action Plan (MAP)

o    Implement and submit a Daily Progress Report (DPR) for Tier 2/Secondary Interventions

o    Use and submit a Tier 2/Tier 3 Interventions Tracking Tool which includes students with:

§  Simple Tier 2 interventions (CICO or SAIG)

§  Tier 2 interventions with individualized features

§  Highly individualized Tier 3 interventions (Complex FBA/BIP OR wraparound/RENEW/Family Focus)

o    Score 70 or higher on the Benchmarks of Quality (BoQ), or score 80/80 on the School-wide Evaluation Tool (SET)

o    Use and submit the Self-Assessment Survey (SAS)

o    Use and submit the Benchmarks for Advanced Tiers (BAT)

Fully Implementing: Sustaining school-wide PBIS implementation, maintaining Tier 2 Interventions, and building the full continuum of supports for Tier 2/Tier 3

Requirements:

·         Provide one-year of multi-tiered outcome data indicating that students have responded positively to PBIS implementation, demonstrate effectiveness of PBIS across all three tiers, and can be reported through the SWIS Triangle Data Report from the Year End Report or other comparable documentation:  Tier 1/Universal behavior supports - 85% or more of all students have 1 or fewer office discipline referrals/ODRs, Tier 2/Secondary behavior supports - 10% or less of all students have no more than 2-5 ODRs, Tier 3/Tertiary behavior supports - 4% or less of students have 6 or more ODRs, and 5% or less of students have received out-of-school suspensions, and a small number of students are placed out of their home school.  Note:  Additional evidence and/or follow-up conversations may be necessary to fully evaluate this discipline data.

·         Use and submit the PoI at least once yearly

·         Demonstrate PBIS implementation by fulfilling all criteria on the most recent PoI at the following phases: Tier 1, Phase I, II, and III, Tier 2, Phase I, II, and III, and Tier 3, Phase I and II

Provide the following artifacts (see application for details) demonstrating implementation of each relevant phase in the PoI:

Exemplar: Demonstrating the “ideal model” of building, maintaining, and sustaining Tier 1,Tier 2, and Tier 3 school-wide PBIS, while maintaining academic integrity

Requirements:

·         Provide two years of multi-tiered outcome data indicating that students have responded positively to PBIS implementation, demonstrate effectiveness of PBIS across all three tiers, and can be reported through the SWIS Triangle Data Report from the Year End Report or other comparable documentation:  Tier 1/Universal behavior supports - 85% or more of all students have 1 or fewer office discipline referrals/ODRs, Tier 2/Secondary behavior supports - 10% or less of all students have no more than 2-5 ODRs, Tier 3/Tertiary behavior supports - 4% or less of students have 6 or more ODRs, and 5% or less of students have received out-of-school suspensions, and a small number of students are placed out of their home school.  Note: Additional evidence and/or follow-up conversations may be necessary to fully evaluate this discipline data.

·         A copy of documentation that demonstrates 80% of students are Meeting/Exceeding standards in Reading and Math on the ISAT for FY12 OR a copy of documentation that demonstrates school is maintaining or making progress toward at least one school improvement academic goal (student outcome data).

·         Use and submit the Illinois Phases of Implementation Tool (PoI) at least once yearly

·         Demonstrate PBIS implementation by fulfilling all criteria on the most recent PoI at the following phases: Tier 1, Phase I, II, and III, Tier 2, Phase I, II, and III, and Tier 3, Phase I, II, and III

Provide the following artifacts (see application for details) demonstrating implementation of each relevant phase in the PoI:

o    Use and submit a Multi-Tiered Action Plan (MAP)

o    Implement and submit a Daily Progress Report (DPR) for Tier 2/Secondary Interventions

o    Use and submit a Tier 2/Tier 3 Interventions Tracking Tool which includes students with:
