Accessing the Inventory
WHEN: On March 21, 2014, all schools in Illinois will have access to the Inventory through the test site. Schools are encouraged to respond to the Inventory every 3rd or 4th team meeting until 80% is reached at all three Tiers. Coaches and teams should have a plan in place for how often a team member is entering data.
If your school wishes to be Recognized by the Illinois PBIS Network for efforts during the 2013-14 school year, your school will need to take the Inventory at least once in the test site by May 16th, 2014. The Illinois PBIS Network will look to these entered Inventory responses to award Recognition levels for the 2013-14 school year. More information about Recognition can be found here: http://www.pbisillinois.org/schools/school-recognition. Schools do not need to send their Inventory Reports to the PBIS Network- we have access to the scores.
Any school that self-assessed with the Implementation Inventory during the beta test period meets the Inventory criteria for Recognition.
WHO: Only 1 team member/coach should enter the responses on behalf of the team (each member of the
team does not need to enter their own responses). Coaches should have a plan in place for who will be responsible for entering the data for all three Tiers into the test site, and how often they will enter the data. Remember, it is suggested to submit the Inventory every 3rd or 4th team meeting.
Beta Test Group Please Note: The tool name has been changed from Implementation Inventory to School-wide PBIS Fidelity Inventory. If you took the Inventory during the beta test period before March 13th, when you log in next you'll notice some items on the Inventory will be slightly different. Features have been updated, and though the changes are relatively minor they will impact data entry going forward. The changes to the Inventory were made in light of the collective experiences and suggestions from groups across the US who were involved in the beta test. Therefore, going forward, please use the updated version of the Inventory tool and guide (available for download at the top of this page), and distribute to your teams. Any data entered online under the previous version during the beta test period will still be accessible, and will remain in the system under the "School-wide PBIS Implementation Inventory" Survey.
Directions
Follow the directions below to enter your team’s responses to the Inventory online.
REQUESTING ACCESS:
1. If you are not a district coach, and did not attend an Inventory webinar in Dec/Jan, before attempting to login to the test site you will need to request to be added as a Respondent for your school(s). Use this form:

2. Once you have access as a Respondent, using Google Chrome as your web browser, go to: https://fieldtest.pbisassessment.org/Login
LOGGING IN:
3. Use your login credentials to log into the test site. Your login information is as follows:
If you are a user in your school’s SWIS account, use your same username (email address) & password to access this test site.
If you are your district’s PBIS Assessment Coordinator, use your same username (email address) & password to access this test site. (If you are a PBIS Assessment Coordinator, this means you open & close surveys for the schools you coach.)
WHY: The Inventory is currently housed in the test site of PBIS Apps. If you take the Inventory in the test site during spring 2014, it will
automatically transfer to your school’s PBIS
Assessment account in the live PBIS Apps website, in the fall of 2014.
WHAT: The School-wide PBIS Tiered Fidelity Inventory(previously named Implementation Inventory) provides a reliable measure of the extent to which school personnel are applying the core features of school-wide positive behavioral interventions and supports at Tier 1, Tier 2, and/or Tier 3. The Inventory is divided into three sections that can be used separately or in combination to assess the extent to which core features are in place. For more information about the purpose of the tool, please reference the guide, page 4.

If you have forgotten your password to either of the above accounts, you can use the “Forgot Password?” link, located below the green login button on the test site home page, and you will receive a prompt on how to reset your password.
All District Coaches on record with the Network have already been added to their districts for data entry. They will be emailed their login information if they aren't already a SWIS user or PBIS Assessment Coordinator.
In order for each school to enter their own Inventory online, please follow this link to provide a name and email for one person from each school who will enter the results: https://docs.google.com/forms/d/1hln0bi-TvRvf3ahH7Dcpdiaz4YpcIoks6XOsoQ_imB4/viewform
Please allow up to 72 hours for us to set up these accounts. The school contact will then receive instructions and the link for the Inventory. Please make these arrangements as soon as possible, or no later than May 1st, to give schools time to use and enter the Inventory.
ACCESSING THE INVENTORY AFTER LOG IN:
4. After you have logged into the test site, you will see the “Surveys” option along the gray bar at the top of the page (the menu). Click on “Surveys”.
5. Once you have selected “Surveys” from the menu, a new page will appear. Select “School-wide PBIS Tiered Fidelity Inventory” from this new page.
Your page will then display all the school(s) you have access to.
6. Locate your school name under the “Organization” column displayed on this page, and go over to the “Take Survey” link.
If you do not see your school name listed, please email eval@pbisillinois.org with the name(s) of the school you need access to, and we will get them added to your account in the test site.
7. Now that “Take Survey” is selected, your Inventory will load on screen, and you will be prompted through each question.
*updated* Note: Your progress is saved automatically for 7 days only if:
the responses are entered by the same user login,
responses are entered for all three Tiers during that login session. Your answers will not show up in reports until the entire survey has been submitted.
Therefore, it is recommended that for the spring, you enter all three Tiers during one sitting, under one user login, to avoid saving issues.
You are required to enter data for each item, at every Tier. If you school does not have a Tier 2 or Tier 3 team in place, you would appropriately enter “0=Not implemented” for any item.

REPORTING:
8. To find your score(s) at each tier you have two options:
Manually score your Inventory using the paper Inventory tool. (Refer to page 23 in the guide).
Use the Generating an Inventory Report document to create a Subscale Score, Sub-Subscale Score, or Items Scoring Report.
LOGOUT:
9. To logout of the test website, go to “Your Account” from the menu, then go down to “Logout”.
PROBLEMS/CORRECTIONS:
If you encounter any issues while using the test site to enter your Inventory responses, use the "Report A problem" hyperlink located on the bottom of the test site Login page.
